How to Apply for Services
Individuals and families seeking to utilize resources must fill out an application in person at The Collective and provide a photo ID along with any additional documentation relevant to the services requested.
Client consultations are scheduled by appointment: Monday through Thursday from 10am-2pm.
Each consultation begins by establishing a relationship where the client may describe their current circumstances and the services they are requesting in a confidential environment. Working together, we then identify the obstacles and what assets each person and The Collective has to share in overcoming those obstacles. Finally, we build an individualized client-agency partnership agreement with a quantifiable plan of action setting achievable goals.
The Collective’s policy is to facilitate weekly progress toward the achievement of planned goals and note periodic updates on client status.
